Perioperative Medicine Fellowship

Overview
Prince of Wales Hospital is a tertiary referral centre with a full range of surgical sub-specialties providing state-of-the-art treatment for cancer, including minimally invasive robotic upper GI, lower GI, hepatobiliary, urological and gynaecological procedures, ENB thoracic procedures, cytoreductive surgery, HIPEC and PIPAP, to name a few.
A team of dedicated anaesthetists work in close collaboration with surgeons, physicians, nursing and allied health professionals to perform risk assessment, counselling and implement prehabilitation programs for high risk surgical patients.  ERAS principles have become fully embedded into our training and practice and guide our day-to-day intraoperative management.  Our pain consultants and pain nurses run a comprehensive acute pain service to facilitate early return to function and mobility.

List of perioperative services include:

  • Pre-op Assessment Clinics (POAC)
  • Prehabilitation Clinics for high risk patients, incorporating fragility screening, cognitive function screening, nutritional status screening and exercise capacity screening
  • Comprehensive outpatient prehabilitation program
  • Anaesthetist-led Echo clinics 
  • Cardiopulmonary Exercise Testing (CPEX) clinics led by POETTS accredited CPEX practitioners and trainers
  • Anaemia Clinics and preoperative intravenous iron therapy
  • Counselling and shared decision making

  • Research and Audits

Fellowship opportunities:
During the 12-month fellowship, fellows will take part in providing all of the above out-patient perioperative services under the guidance of a passionate team of consultant anaesthetists.  Our fellows will gain experience in performing comprehensive risk assessment, fragility screening, managing preoperative anaemia; and be guided to acquire proficiency in performing focused echo and interpreting CPEX results. Clinical time will also be allocated to looking after high risk patients undergoing a variety of cancer surgeries with input or supervision catered to the fellow’s individual needs and stage of training.  Furthermore, our fellows will have the opportunity to collaborate with a multidisciplinary team throughout the training, engage with various stakeholders of a prehabilitation program, and leave the program with the skills required to develop or lead a perioperative service. 
The fellow will be provided with the opportunity to take part in weekly continuous medical education activities, present at academic meetings, as well as embark on a quality improvement/audit or research project.

What to expect:
Clinical time will be divided between outpatient assessment and optimization clinics and the operating theatres.  Fellows are also expected to provide 3-4 calls per month.  Local medical and nursing staff are fluent in English and are able to help with translation and interpretation if needed.  Our fellows will be provided with protected time to participate in quality improvement and research projects, and to provide teaching for anaesthesia trainees. All fellows are allocated mentors throughout the program.
