Octreotide

Indications

Established indications

  • pituitary adenomas (GH and TSH secreting tumours which recur following surgery)
  • pancreatic islet cell tumours
  • metastatic carcinoid tumours
  • scintographic imaging of somatostatin receptor positive tumours

Probable indications

  • bleeding oesophageal varices
  • pancreatic and enteric fistulas
  • complications of elective pancreatic surgery
  • secretory diarrhoea

Unestablished indications

  • somatostatin receptor positive neuroendocrine tumours
  • somatostatin receptor positive adenocarcinomas
  • ERCP related pancreatitis
  • upper GI bleeding

Complications

GI

  • dose dependent symptoms of nausea, cramps, fat malabsorption and flatulence
  • appear following first administration but disappear after 1-2 weeks of treatment

Metabolic

  • reduced glucose tolerance and hyperglycaemia (rarely a problem)
  • cholesterol gallstones are more common in patients taking octreotide for > 1 month
