Slow Low Efficiency Daily Dialysis

  • variant of intermittent haemodialysis
  • duration of dialysis 8-12 hours
  • compared to conventional intermittent haemodialysis:
    • lower blood flow
    • fluid removal & solute clearance slower
    • less haemodynamic instability
  • excellent solute control
  • not associated with significant urea dysequilibrium when used over 12 hours
  • commonest complication is clotting of circuit so many patients require anticoagulation:
    • 1000 U unfractionated heparin bolus, followed by 500 U/hour – significant bleeding not reported
  • other complications include:
    • mild hypophosphataemia and hypokalaemia
    • some amino acid loss
    • minimal albumin loss
  • less effective solute clearance than CVVH, particularly for middle and large molecules
  • nocturnal scheduling of SLEDD allows other procedures (eg CT, surgery) to be carried out during the day. Can also free up machine for intermittent haemodialysis during the day
  • less expensive than most CRRT
