Epidemiology

  • Endemic in South east Asia

  • Reported in Thailand, Indonesia, Hong Kong

  • High mortality

  • 3rd most common opportunistic infection in AIDS patients in Thailand other than extrapulmonary TB and cryptococcosis

Clinical features

  • fever

  • malaise

  • weight loss

  • anaemia

  • lymphadenopathy

  • hepatosplenomegaly

  • bilateral lung infiltrates

  • papules on skin with central umbilication

Diagnosis

Skin , lymph node biopsy, blood culture, bone marrow culture

Treatment

  • IV amphotericin 0.6 mg/kg x 2 weeks

  • Itraconazole 200 mg bd x 6-10 weeks
