Diabetic ketoacidosis

The patient was treated with rapid infusion of normal saline and simultaneously given an IV bolus of 8 units of short acting insulin.

30 minutes later she developed this arrhythmia

What is the likely cause for the arrhythmia and was the initial management of her diabetic ketoacidosis appropriate?
